About The Position

Community Care is hiring a Program Assistant/CNA to join their team. This is a part-time, 24 hours per week position for the Home Care department at their Waukesha location. The role is ideal for someone who enjoys team collaboration, is mission-driven, and shares the organization's goal of helping elderly individuals and adults with physical and intellectual disabilities live independently in the community.

Requirements

  • A minimum of 1 year of experience providing care or services for frail elderly and individuals with physical or developmental disabilities.
  • Wisconsin Certified Nursing Assistant (CNA) in good standing on registry.
  • CPR & AED certification.
  • High School Diploma or GED.
  • Valid Wisconsin Driver's License.
  • Proof of required insurance coverage (for travel to members' homes).

Nice To Haves

  • Experience working with individuals with dementia.

Responsibilities

  • Under the supervision of the assigned Day Center Supervisor or designee, provide care and services for the personal, recreational, support, and health needs of Day Center attendees.
  • Run recreation activities, leading and supervising group and individual activities, projects, and special events under the direction of Recreation Specialists and/or supervisors.
  • Assist in the daily cleanup of recreation areas.
  • Deliver written restorative programs, including leading therapeutic exercise groups, and complete related documentation.
  • Stay informed of member care plans and provide restorative and personal care as outlined.
  • Stay informed of member care plans, update the team on member changes (physical, emotional, or behavioral), and recommend changes.
  • Provide various care services necessary for attending to the personal needs, support, and health status of Community Care program members in their homes.
  • Occasionally or regularly travel to members' homes or other residential facilities, requiring the use of a personal vehicle, valid driver's license, and proof of insurance.
