About The Position

The REACCH (Resources, Education and Comprehensive Care for HIV) Program provides comprehensive medical care and social services to people living with HIV throughout south central Pennsylvania. The REACCH program provides an array of services including HIV primary and specialty care, case management and social work services, dental care, on-site therapy/counseling, rapid HIV testing, community outreach and education, and HIV prevention services. As a Professional Staff Nurse, you will play a vital role in delivering compassionate, high-quality patient care as part of a collaborative interdisciplinary team. Using clinical expertise, critical thinking, and evidence-based practice, you will coordinate and provide care for patients and their families while fostering meaningful partnerships with physicians, providers, and support staff. Requirements

Minimum six months experience preferred.
The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit.
They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures.
Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
Graduate nurses must complete licensure examination within SIX MONTHS of hire or within one year of graduation, whichever comes first.

Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state.
Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.

Nice To Haves

BSN preferred.
Interest in public and community health is a plus.
Experience working with populations with multiple social needs is beneficial.
UPMC approved national certification preferred.

Responsibilities

Deliver comprehensive, patient-centered nursing care that promotes quality outcomes, patient safety, and satisfaction.
Collaborate with physicians, advanced practice providers, and interdisciplinary team members to coordinate care and develop individualized plans of care.
Assess, triage, and manage patient needs while utilizing sound clinical judgment and critical thinking skills.
Assist providers with patient assessments, treatment plans, procedures, education, and follow-up care.
Identify and address clinical, social, psychological, safety, and spiritual factors that may impact patient health and well-being.
Educate patients and families on diagnoses, treatments, medications, and self-care needs through in-person, telephone, and written communication.
Maintain accurate and comprehensive clinical documentation that supports continuity of care and effective communication among providers.
Serve as a patient advocate by helping individuals navigate healthcare needs, barriers to care, and available resources.
Participate in quality improvement initiatives and utilize evidence-based practice to enhance patient care, safety, and outcomes.
Support the onboarding, development, and mentorship of students, new staff, and colleagues while fostering a culture of learning and professional growth.
Actively engage in shared governance, process improvement initiatives, and departmental goals aimed at advancing professional nursing practice.
Build and maintain positive working relationships with patients, families, providers, and team members to create a compassionate and respectful care environment.
Demonstrate accountability for professional development, lifelong learning, and maintaining clinical excellence.
