About The Position

Reporting to the Director, Home & Clinical Care, this position is responsible for ensuring the adoption of evidence-based best practices, aligning their approach with current professional standards, regulatory requirements, and practice expectations relevant to their assigned discipline – Nursing, Allied Health, and/or Personal Support Services – on behalf Carefor’s Home and Clinical Care teams. As a critical resource to the Home & Clinical Care teams, the Professional Practice Lead will offer advice and expert consultation towards ensuring safe, client-centered practice. Setting practice standards, defining care processes and procedures, designing education and training, and evaluating care delivery models for target client populations are key areas of focus for the position. In collaboration with analytics and business intelligence, the use of data and evidence to inform delegated care and care delivery will be used to advance the Home & Clinical Care team’s practice priorities in sustaining best practice guideline implementation. Striving for excellence in quality client care, the position provides leadership and coordination in the development of evidence-based practice projects, programs and initiatives for the Home & Clinical Care teams. This is a 6-month term position.

Requirements

• Bachelor of Science in Nursing (BScN) and registration in good standing with the College of Nurses of Ontario (required)
• Minimum of three (3) years recent experience in the Adult Home and Community Care sector with demonstrated advanced practice expertise supporting Nursing, Allied Health and/or Personal Support Care for older adult population.
• Demonstrated knowledge of applicable Ontario legislation, regulatory requirements, and professional practice standards.
• Demonstrated ability to promote and facilitate evidence-informed practice in community settings, with expertise in teaching, coaching, mentoring, and supporting individuals and teams.
• Excellent communication and interpersonal skills, with the ability to influence, engage, and, motivate others while maintaining a credible and professional presence across all levels of the organization.
• Demonstrated critical thinking, analytical, decision-making, and problem-solving skills.
• Excellent organizational, planning, prioritization, and time management skills.
• Proficiency in using Microsoft Office applications including Outlook, Word, Excel, and PowerPoint.
• Regular travel throughout the Champlain region is required to support Nursing, Allied Health, and/or Personal Support Services team members in a variety of community settings, including private homes, retirement residences, and Carefor clinics.
• Participation in a leadership on-call rotation, including evening, weekend, and statutory holiday coverage, is required.

Nice To Haves

• Candidates currently pursuing a nursing degree and/or professional registration who possess significant healthcare leadership, professional practice, education, and quality improvement experience, supported by other relevant credentials, will be considered.
• Master’s degree in Nursing, Health Sciences, Education, Leadership, or a related field (preferred)
• Certification in clinical specialty, chronic disease management, or related areas (e.g., Stroke, Parkinson’s Disease, Multiple Sclerosis, ALS, ADP Authorizer) is considered an asset.
• Bilingualism (French/English) is preferred.

Responsibilities

• Provide professional practice leadership and subject matter expertise to support the development, implementation, evaluation, and continuous improvement of professional practice frameworks, delegated practice programs, policies, procedures, and innovative service delivery models across Nursing Services, Allied Health, and/or Personal Support Services.
• Lead evidence-informed practice change initiatives that advance clinical excellence, promote innovation, and support high-quality, person-centered care
• Develop, implement, and evaluate education programs, learning resources, orientation processes, and professional development initiatives that support clinical competence, regulatory requirements, and evolving practice needs.
• Provide mentorship, coaching, consultation, and guidance to leaders and frontline staff, fostering professional growth, clinical confidence, accountability, and a culture of continuous learning.
• Lead and support quality improvement, patient safety, risk management, and performance improvement initiatives that strengthen practice standards, improve client outcomes, and promote the effective use of resources.
• Analyze and apply evidence, outcome data, and emerging best practices to identify opportunities for improvement and enhance quality of care and quality of life outcomes.
• Partner with operational leaders, interdisciplinary teams, and internal and external collaborators to design, implement, and evaluate programs and care delivery approaches that meet organizational and client needs.
• Foster collaborative relationships and contribute to a culture of clinical curiosity, shared accountability, inclusion, and continuous improvement across teams and services.
• Monitor and interpret relevant legislation, regulations, accreditation standards, and professional practice requirements to ensure compliance and inform practice, policy, and program development.
• Contribute to organizational priorities, strategic initiatives, and business development opportunities that support sustainable growth, service excellence, and positive client and organizational outcomes.
• Support the Director, Home & Clinical Care, and operational leaders in achieving service, quality, and performance objectives across assigned programs and services.
• Provide leadership coverage and decision-making support as required to ensure continuity of operations and effective service delivery.
• Serve as a clinical and professional resource to leaders and staff by providing coaching, mentorship, education, and consultation that supports individual growth, clinical excellence, and operational effectiveness.
• Support staff development through the identification of learning needs and the implementation of professional development opportunities aligned with evidence-informed practice, professional standards, and organizational expectations
• Partner with managers and supervisors to support performance management activities, including performance appraisals, goal setting, recognition, performance improvement planning, and employee relations matters.
• Provide guidance and targeted interventions to address identified practice concerns, support staff success, and promote accountability and continuous improvement.
