Requirements
- Possession of a current Louisiana license to practice as a Provisional Licensed Professional Counselor (PLPC) or a Provisional Licensed Marriage and Family Therapist (PLMFT).
Responsibilities
- Conduct comprehensive substance abuse assessments for individuals referred by DCFS.
- Develop individualized treatment plans based on assessment findings an client needs.
- Collaborate with community agencies and service providers to ensure clients receive ongoing support and continuity of care.
- Maintain accurate client data and prepare monthly and annual reports required by DCFS.
- Participate in multidisciplinary team meetings and case staffing to coordinate comprehensive client care.
- Provide case management services by coordinating referrals and connecting clients with community resources and ancillary/recovery support services.
- Educate clients and their families about addiction, recovery, relapse prevention, and available community resources.
- Coordinate with administrative support staff to reschedule appointments for clients who missed scheduled visits.
- Complete clinical assessments, treatment documentation, progress notes, and other required records in a timely and accurate manner.
