Requirements

Master’s degree in Social Work (MSW) from an accredited program and a licensure for the state of Indiana
Minimum of one-year of experience in social work, grief counseling, bereavement support or related in a healthcare field, preferably in hospice or palliative care.
Clean driving record, and valid driver’s license and auto insurance

Candidates with experience in grief counseling, bereavement support, and community outreach are encouraged to apply.

Responsibilities

Comprehensive Psychosocial Assessment
Counseling and Supportive Interventions
Advance Care Planning and Decision Support
Resource Referral and Coordination
Bereavement Support and Follow-up
Provide comprehensive psychosocial support to patients and families facing end-of-life issues.
Assist in providing comprehensive bereavement support to families and caregivers following the death of a hospice patient as needed.
Travel as necessary within the designated area.
Any other duties as assigned by leadership.
