Our Home Care Teams proudly provide the best possible care to our patients throughout the Cleveland and Northeast Ohio area. As an RN on the Home Care Team, you will provide services to those living primarily in Cleveland – east, west and the downtown district. Further coverage will expand as far west as Vermillion. In this role, you will provide assessments, complex treatments and education to help avoid readmissions to the acute care setting and ensure comfort. This position offers a flexible schedule, allowing you to maintain a healthy work-life balance, all while changing the lives of your patients and their loved ones every day. This is a PRN role. A caregiver in this position will work flexible day or night shifts, including 4-, 8-, or 12-hour options. Day shifts run between 8:00 a.m. and 8:30 p.m. or 9:00 a.m. and 9:30 p.m., while night shifts may range from 3:00 p.m. to 3:30 a.m. or overnight hours. Weekend coverage is required one shift per six-week period, with scheduling for one holiday per year.

Requirements

  • Graduate from an accredited school of Professional Nursing
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross
  • Minimum one year current clinical experience as a registered nurse
  • One year of clinical experience as an RN in an acute medical/surgical setting
  • Valid Driver’s License (VDL) in Ohio and proof of automobile insurance with $100,000/$300,000 coverage

Responsibilities

  • Provide direct nursing care in accordance with established healthcare organization policies, procedures, and protocols.
  • Implement and monitor patient care plans.
  • Provide supervision to LPNs, Home Care Aides and families/caregivers involved in the care of the patient.
  • Perform accurate and complete admission assessments and reassessments within a specified time frame.
  • Assess patient for safety risks, abuse and neglect.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Assess and document patients' pain using appropriate pain scale and manage patients' pain then.
  • Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Escalate change in patient condition to patient’s physician as appropriate.
