This position could work during the day throughout the week or cover on-call nights and weekends occasionally.

Requirements

• RN licensure for the state of Indiana
• Clean driving record, and valid driver’s license and auto insurance

Responsibilities

• Provide comprehensive care coordination and support to hospice patients and their families.
• Assess patient needs, develop individualized care plans, coordinate care services, and provide emotional support throughout the end-of-life journey.
