The current opening is for an Advanced Practice Provider (FNP, NP, Adult Gero NP) or Physician Assistant in Fremont, OH, to serve local skilled nursing facilities within a close-knit, mission-driven care community.
Requirements
- Hold an active U.S. license as an Advanced Practice Provider
- Be board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants)
- Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine
Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities
- Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
- Communicate with facility nursing staff via phone or secure messaging as needed
- Maintain reliable on-site attendance for scheduled shifts
- Ensure compliance with clinical standards, facility policies, and regulatory requirements
