Requirements

Master's Degree with appropriate licensure / certification as per State regulations
At least two (2) years of clinical experience in mental health field (additional training may substitute for one year)
Licensed in the state of Indiana with one of the following: LCACA (Licensed Clinical Addiction Counselor Associate), LMSW (Licensed Masters Social Worker), APC (Associate Professional Counselor), AMTF (Associate Marriage &Family Therapist)
Demonstrates proficiency in communication and written skills
Knowledge of State and Federal statutes regarding patient confidentiality laws
Knowledge of licensing and accreditation standards

Preferred License LPC (Licensed Professional Counselor)
LCSW (Licensed Clinical Social Worker)
LMFT (Licensed Marriage & Family Therapist)
LCAC (Licensed Clinical Addictions Counselor)

Responsibilities

Performing a wide variety of patient care activities as directed by the Clinical Director.
Responsible for patient caseload.
Providing group, individual, and family therapy.
Conducting psychosocial and biopsychosocial assessments.
Collaborating with the Treatment Team to formulate the Master Treatment Plan.
Assisting in coordinating discharge planning.
Primary responsibility for maintaining communication with the family, referral source and other service providers as needed.
