Requirements

  • 1 year of home health, private duty, healthcare scheduling, or related experience preferred.
  • Strong communication, organization, and attention to detail.
  • Ability to multitask, problem-solve, and stay calm in a fast-paced environment.
  • Comfortable working independently and as part of a team.
  • Commitment to compassionate, patient-centered care.

Responsibilities

  • Coordinate daily schedules for private duty patients and caregivers, ensuring appropriate coverage based on patient needs and caregiver availability.
  • Match qualified caregivers with patient assignments based on skills, availability, location, and patient needs.
  • Communicate with patients, families, caregivers, and clinical teams regarding schedules, changes, and coverage.
  • Manage call-outs, cancellations, and schedule changes while minimizing disruptions to patient care.
  • Maintain accurate schedules, patient information, caregiver assignments, and service documentation.
  • Assist with new patient starts and ensure caregivers have the information needed to provide safe, compassionate care.
