The Role We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide care for an extended care client on a PRN basis during day shift 0700-1900 or night shift 1900-0700. This is a private duty client with trach, vent, G-tube, and Hoyer transfer needs in addition to brief changes, bath cares, and med administration. You will be working in the patient's home, ensuring their safety, comfort, and medical stability.

Requirements

  • Current, unencipumbered LPN or RN license
  • Experience with trach, vent, and G-tube care
  • Current BLS/CPR certification
  • Strong clinical and communication skills
  • Ability to work independently in a home care setting
  • A compassionate, patient-focused approach to nursing
  • Must work at least 1 shift/month to remain PRN on the case.

Responsibilities

  • Delivering specialized care for a patient with trach, vent, and G-tube
  • Monitoring the patient's respiratory and nutritional health during your shifts
  • Administering medications and tube feedings
  • Performing routine assessments, interventions, and adjustments as necessary
  • Communicating with the care team and maintaining accurate patient records
  • Developing strong, supportive relationships with the patient and their family
