Requirements
- Active LPN or RN License issued by NYS
- Minimum of 1 year experience in a home care setting.
- Current CPR/BLS Certification
- Empathy, Reliability, and a commitment to high-quality care
- Must be able to lift 50lbs.
- Must be able to stand/work for extended periods
Responsibilities
- Perform 1:1 patient care duties and treatments as per care plan.
- Provide health promotion, counseling, and education.
- Administer medications and other personalized interventions.
- Coordinate care, in collaboration with a wide array of health care professionals.
- Document nursing notes regarding patient care daily.
