About The Position

Our Home Health Aides provide compassionate care to patients in their homes, supporting their independence and assisting with activities of daily living. The role includes monitoring patients’ vital signs and physical or mental condition, assisting with grooming and hygiene, and helping with household tasks so clients can maintain the familiarity and dignity of living at home. Home Health Aides support each patient’s Plan of Care, communicate with the care team and family, and help families understand patient care and aftercare needs.

Requirements

High school graduate or equivalent.
Home Health Aide Certificate or similar certification in good standing from an accredited training course that meets Health and Human Services standards.
At least one year of experience as a home health aide or certified nursing assistant (CNA).
Strong written and verbal English-language skills.
Active CPR certification.
Valid driver’s license, reliable transportation, and insurance.
Effective verbal and written communication skills.
Relationship management and ability to influence behaviors.
Knowledge of the healthcare environment, regulatory requirements, and evidence-based practice.
Excellent procedure skills and clean/sterile technique.
Exemplary assessment skills.
Physical stamina.

Responsibilities

Support the patient’s Plan of Care by monitoring and recording condition, providing personal services, and teaching families.
Observe and assess physical and mental condition, intake and output, and exercise; collaborate with the multidisciplinary care team to develop and implement a plan of care.
Communicate and partner effectively with the designated care team to support continuity of care.
Provide housekeeping, laundry, shopping, other household tasks, and meal and snack preparation and service.
Provide personal care as needed, including bathing, dressing, and grooming.
Teach family members appropriate ways to lift, turn, and reposition the patient.
Address family members’ concerns and questions and educate them about the patient’s treatment and aftercare needs.
Record patient information in the EMR system and notify branch leadership of changes or unusual conditions.
Take ownership of requests and explore opportunities to add value to services.
Perform on-call responsibilities and provide on-call services to patients and families if assigned.
Maintain a safe, secure, and healthy environment by following asepsis and universal precaution controls during client care.
Follow professional standards, home care policies and procedures, and applicable federal, state, and local requirements.
Maintain mandatory continuing education requirements (CEUs) of the agency and licensing board.
