About The Position

The Primary Nurse - PACE is responsible for the general oversight, assessment, planning, and construction of the nursing care delivered to the participant. This role involves assessing patients by obtaining history, performing appropriate tests, making or collecting observations from participants and caregivers, and consulting with other members of the Interdisciplinary Team (IDT). The nurse will teach patients and families about healthcare regimens, explain disease and wellness information, tests, results, medications, diet, exercise, and other aspects of the plan of care. They will safely and effectively administer medications, tests, and treatments as ordered, and manage medication refills with provider authorization. The role also includes providing and coordinating health teaching for individuals, families, and groups to maintain optimal health levels. The nurse will complete biannual and status change nursing assessments for an assigned panel of participants, documenting problems, goals, and interventions in the participant’s care plan, utilizing assessment tools such as the PACE Nurse Assessment and Medication Management.

Requirements

• Current Massachusetts license in nursing.
• Current BLS certification required.
• A minimum of one year experience working with a frail or elderly population.

Nice To Haves

• Baccalaureate degree in nursing preferred.
• Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred, equivalent training or related experience considered.
• Prior experience in interdisciplinary team setting a plus.

Responsibilities
