We are seeking a compassionate and dynamic Primary Care Supervisor to join our Atlas Medical Team. A Primary Care Supervisor is responsible for maintaining and growing relationships with existing referral sources and customers such as hospitals, physician offices, case managers, discharge planners, long-term care facilities, and home health agencies. The focus is on account retention, service quality, and revenue growth within assigned territories.

Requirements

• Requires 2–5+ years in management / supervisor role.
• Strong leadership and communication skills.
• High attention to detail and accountability.
• Ability to multitask and learn multiple workflows.
• Possess a valid driver's license, clean driving record, and reliable automobile.
• Provide proof of current automobile insurance (MVR).
• Have a clear negative TB test.

Responsibilities

• Supervise all Atlas departments and employees.
• Conduct regular one-on-one meetings with team members.
• Review outcomes with the Atlas Practice Manager.
• Hold team members accountable for workflows and deadlines.
• Learn all department roles and workflows.
• Provide coverage during employee absences.
• Identify process gaps and support improvements.
• Manage the Chronic Care Management (CCM) program.
• Create and maintain care plans.
• Meet regularly with Sue regarding CCM programs.
• Monitor facility team threads and ensure timely responses.
• Collaborate closely with the Atlas Practice Manager.
• Handle calls from upset patients or facilities professionally.
