About The Position

At NorthBay Health, the role of the Primary Care Team Registered Nurse (Team RN) is to function as a member of the multidisciplinary team. The Team RN utilizes clinical knowledge and competence, positive communication skills, problem solving and conflict resolution techniques, ability to effect change, strong skills in assessment, organization and time management. The Team RN will be familiar with the case management process requiring a focus on customer service skills, knowledge of setting appropriate goals and measuring outcomes to effectively ensure optimal client outcomes with consideration to payer requirements. The Team RN works in collaboration with his/her respective healthcare teams. The Team RN works across the healthcare continuum encompassing the NorthBay HealthCare System, network and out of network services in collaboration with patients/families, physicians, staff and administration to ensure appropriate care at the appropriate level to the member. The Team RN works under the direction of the Manager and Director of the Centers for Primary Care.
