About The Position

We are an established healthcare continuum care company seeking an entrepreneurial and self-motivated Primary Care Provider to support the continued growth of our mobile primary care division throughout the Phoenix metropolitan area. Both Full-Time and PRN opportunities are available. Full time position with a Monday through Friday schedule from 8:00 AM to 5:00 PM. Our organization focuses on providing high-quality mobile primary care and advanced wound care services while incorporating innovative healthcare technology and patient-centered programs that improve outcomes, including chronic care management, remote patient monitoring, and transitional care management. Through our full continuum care model consisting of mobile primary care, home health, and hospice services, we help patients remain comfortable in their home setting while reducing unnecessary hospitalizations and skilled nursing facility admissions. The ideal candidate will demonstrate strong clinical judgment, servant leadership qualities, and a passion for delivering compassionate, evidence-based care in a mobile setting. Providers must be comfortable working independently while collaborating closely with an interdisciplinary healthcare team.

Requirements

  • Active Arizona Nurse Practitioner license or ability to obtain prior to employment
  • National board certification as a Nurse Practitioner from an accredited agency
  • Active DEA license with prescribing authority
  • Strong interpersonal, organizational, and written communication skills
  • Ability to work independently while collaborating effectively with a multidisciplinary team
  • Strong critical thinking and evidence-based practice skills

Nice To Haves

  • Certified Nurse Practitioner designation preferred
  • One to two years of clinical experience in Family Medicine (preferred)
  • Experience in advanced wound care and skin substitute applications preferred
  • Knowledge of billing and coding preferred

Responsibilities

  • Evaluating and treating patients in assisted living facilities, skilled nursing facilities, memory care units, group homes, retirement communities, and private residences.
  • Perform patient assessments, physical examinations, diagnose and manage acute and chronic conditions, prescribe medications, review diagnostic results, coordinate follow-up care, and maintain accurate and timely documentation in compliance with HIPAA and company standards.
