About The Position

Open Door Health Services is seeking a dedicated and compassionate Primary Care Provider (Physician, Nurse Practitioner or Physician Assistant) to deliver high-quality, patient-centered care. This role involves managing acute and chronic medical conditions while collaborating with an interdisciplinary healthcare team to support whole-person care. As a Federally Qualified Health Center (FQHC), Open Door Health Services provides comprehensive primary, urgent, preventive, and select specialty services, along with social support programs for individuals and families—regardless of their ability to pay. Our patient population reflects diverse backgrounds, life experiences, and healthcare needs. This position is located in Muncie, a mid-sized city in East Central Indiana. Requirements

Commitment to ethical, inclusive, and patient-centered care
Experience working with adults
Understanding of the social determinants of health and their influence on patient outcomes
Strong clinical assessment and diagnostic skills
Knowledge of clinical documentation and diagnostic standards (ICD)
Familiarity with HIPAA regulations and ethical practice standards
Excellent communication, interpersonal, and organizational skills
Ability to work independently while thriving in a team-based, integrated care environment
Comfort using electronic health records (EHRs) and digital assessment tools
Unrestricted license to practice medicine in the State of Indiana (for Physicians)
Federal DEA certification and Indiana CSR (for Physicians)
Board Certified or Board Eligible (for Physicians)
Medical degree from an accredited medical school (for Physicians)
Eligibility for unrestricted privileges at Ball Memorial Hospital (for Physicians)
Strong community health/public health orientation (for Physicians)
Active Indiana Registered Nurse (RN) license (for Nurse Practitioners)
Indiana Nurse Practitioner authorization (for Nurse Practitioners)
Federal DEA certification and Indiana CSR (for Nurse Practitioners)
Graduate of an accredited school of nursing (BSN required) (for Nurse Practitioners)
Completion of an accredited Nurse Practitioner program with certification (for Nurse Practitioners)
Possession of Indiana State Physician Assistant License (for Physician Assistants)
National Commission on Certification of Physician Assistant (NCCPA) (for Physician Assistants)
Federal DEA certification and Indiana CSR (for Physician Assistants)
Graduate of an accredited PA program required. Successful completion of an approved Physician Assistant program, testing, and certification (for Physician Assistants)
