About The Position

HealthFinders Collaborative (HFC) is seeking a compassionate, skilled, and collaborative Primary Care Provider to join their team. This role involves providing comprehensive primary medical care to patients across the lifespan, including children, adolescents, adults, and older adults. The provider will work within a holistic, community-based model that emphasizes trust, relationships, and accessibility to healthcare for individuals facing barriers. The position is part of an interdisciplinary team that includes medical, dental, and behavioral health providers, community health workers, and support staff. The ideal candidate will be flexible, creative, compassionate, and possess sound clinical judgment, valuing the contributions of all team members and communicating effectively to navigate complex situations. The role requires a commitment to patient-centered, culturally responsive, and evidence-based care, aligning with HFC's mission and clinical standards.

Requirements

Current Minnesota professional licensure, or eligibility for Minnesota licensure, authorizing the applicant to provide comprehensive primary medical care.
Education, certification, training, and demonstrated clinical competence to care for patients across the lifespan, including pediatric and adult patients.
Ability to assess, diagnose, treat, prescribe, and manage preventive, acute, and chronic health conditions within the provider’s legal scope of practice.
Eligibility for credentialing and enrollment with HFC’s applicable healthcare payers.
Current BLS certification.
Current DEA registration, or eligibility to obtain registration before beginning responsibilities that require it.
Strong knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology, preventive care, and the management of acute and chronic health conditions.
Ability to maintain timely and accurate clinical documentation and meet applicable regulatory, reimbursement, and quality requirements.
Strong organizational skills and the ability to prioritize, respond appropriately to urgent needs, and manage multiple responsibilities.
Ability to use an electronic health record, patient portal, email, and other technology needed to carry out the position’s responsibilities.
Sufficient English-language proficiency to communicate effectively with patients, staff, and partners and to complete clinical documentation.

Nice To Haves

Current board certification or board eligibility appropriate to the position’s scope of primary care practice.
At least two years of experience providing primary care across the lifespan.
Experience in community health, a safety-net setting, or another resource-limited clinical environment.
Experience serving patients who are uninsured or underinsured, immigrants or refugees, or who experience other barriers to healthcare.
Experience working with interpreters and providing linguistically and culturally responsive care.
Experience working within an interdisciplinary healthcare team.
Spanish, Somali, or another language commonly spoken by HFC patients is valued but not required.

Responsibilities

Provide comprehensive primary care to patients across the lifespan in accordance with HFC protocols, evidence-based practice, and current standards of care.
Provide preventive care, health maintenance, acute and episodic care, chronic disease management, reproductive healthcare, and patient education within the provider’s scope of practice.
Assess, diagnose, treat, and monitor patients, referring them for specialty or higher-level care when appropriate.
Provide clinical follow-up and coverage for HFC patients, including prescription refills, laboratory orders and results, abnormal findings, and urgent patient concerns.
Support continuity of care for patients seen by volunteer or other HFC providers.
Develop care plans that consider the clinical, cultural, social, and economic factors affecting each patient’s health and ability to access care.
Maintain timely, accurate, and complete documentation in the electronic health record.
Complete workers’ compensation, insurance, disability, and other patient-related forms as appropriate.
Participate in pre-visit planning, care-team huddles, case consultation, and coordination of patient care.
Communicate with patients through the patient portal, telephone, secure messaging, or other appropriate methods.
Work collaboratively with other HFC providers, staff, contractors, volunteers, and external partners to support coordinated care.
Provide clinical consultation and support to other providers, volunteers, community health workers, and members of the care team.
Offer clear and constructive clinical guidance while supporting staff and volunteers who are developing their skills and growing in their careers.
Recognize and respect the knowledge, experience, and contributions that each team member brings to patient care.
Collaborate across medical, dental, behavioral health, care coordination, and community-based services.
Support clinical learning experiences for students, volunteers, and team members.
Provide community and patient education as requested.
Help build a clinic culture that values learning, accountability, teamwork, and continuous improvement.
Participate in quality-improvement initiatives that strengthen clinical care, patient safety, workflows, protocols, and health outcomes.
Help review, develop, and implement clinical policies and procedures consistent with regulatory requirements and accepted standards of practice.
Support the ongoing training and development of community health workers, clinical support staff, and other members of the health team.
Participate in the review and resolution of patient concerns or formal complaints when clinical input is needed.
Support HFC’s response to occupational exposures or other staff and volunteer health concerns when requested and appropriate.
Follow infection-prevention, workplace-safety, confidentiality, and compliance requirements.
Maintain the continuing education, certification, and licensure requirements necessary to remain in good standing.
Contribute to other clinical and organizational priorities reasonably related to the position and HFC’s mission.
