About The Position

We are an established, rapidly growing healthcare continuum organization seeking an entrepreneurial and highly motivated Primary Care Provider to lead and support our Mobile Primary Care division throughout the Phoenix metro area. This is a full-time, Monday–Friday (8:00 AM–5:00 PM) clinical role focused on delivering high-quality, patient-centered care directly in community and residential settings. Our integrated healthcare continuum includes Mobile Primary Care, Home Health, and Hospice services designed to improve patient outcomes, reduce hospitalizations, and support aging-in-place care. We incorporate: Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Transitional Care Management (TCM), Advanced wound care services, and Coordinated interdisciplinary care. The provider will deliver comprehensive primary care services across multiple settings, including: Assisted living facilities, Skilled nursing facilities, Memory care units, Group homes, Private residences, and Retirement communities.

Requirements

  • Active Arizona license: NP (FNP preferred), DO, or Medical Director eligible
  • Board certification (NP applicants required)
  • Active DEA license with prescribing authority
  • 1–2+ years clinical experience (Family Medicine preferred)
  • Strong clinical judgment and independent practice capability
  • Excellent communication and documentation skills

Nice To Haves

  • Experience in wound care strongly preferred

Responsibilities

  • Conduct comprehensive patient evaluations and physical exams
  • Diagnose and manage acute and chronic conditions
  • Develop and implement individualized treatment plans
  • Prescribe medications and manage ongoing therapies
  • Perform minor procedures (preferred)
  • Wound care, suturing, splinting, skin substitutes
  • Review diagnostic results and coordinate follow-up care
  • Maintain accurate, timely clinical documentation
  • Collaborate with interdisciplinary care teams
