About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Primary Care Physician for a PACE (Program of All-Inclusive Care for the Elderly) organization in Stockton, California. This role focuses on providing comprehensive primary care for older adults enrolled in a value-based PACE program. The physician will lead and collaborate with an interdisciplinary care team to develop individualized care plans, perform comprehensive in-person assessments, and coordinate ongoing medical management. Responsibilities include coordinating transitions of care, conducting scheduled home visits, and maintaining continuity of care. The position offers a manageable patient volume, averaging approximately eight patients per day, allowing for relationship-focused medicine, and does not involve hospital rounding responsibilities.

Requirements

• MD or DO Degree REQUIRED
• Successful Completion of an ACGME Accredited Residency REQUIRED
• Active California Physician License REQUIRED
• Board Eligible or Board Certified in Family Medicine, Internal Medicine, Emergency Medicine, or Geriatrics REQUIRED
• DEA Registration REQUIRED
• California Controlled Substance Registration REQUIRED
• Eligible to Enroll as a Medicare and Medi-Cal Provider REQUIRED
• Commitment to Caring for Older Adult Populations REQUIRED

Nice To Haves

• CPR and First Aid Certification Preferred Upon Hire (Required Within 180 Days)

Responsibilities
