About The Position
Care Guide Partners PACE, part of Hosparus Health, is a Program of All-Inclusive Care for the Elderly (PACE) provider. PACE aims to empower individuals aged 55 and older by offering the necessary support to remain in their homes as they age. This program assists seniors who require nursing home-level care in maintaining their independence by delivering comprehensive medical care and support services, enabling them to live safely and comfortably at home.
Requirements
- MD or DO required.
- Valid MD license in good standing to practice medicine in the State of Kentucky.
- Minimum two years clinical experience required.
- Excellent communication skills, with ability to interact with a wide range of stakeholders, including the IDT Team and external healthcare providers.
- Flexibility to travel throughout the PACE service area.
Nice To Haves
- Fellowship or additional training in Geriatrics preferred.
- Experience working with frail or elderly populations strongly preferred.
Responsibilities
- Provides direct primary medical care to participants of the PACE Program within the Interdisciplinary Team (IDT).
- Supervises clinical medical care provided by Nurse Practitioners, RNs, and Medical Assistants.
