About The Position

We are seeking a Primary Care Physician to join a mission-driven, value-based care model focused on helping vulnerable seniors remain safely in their homes and communities. This is a strong fit for a physician who wants to build deeper, longer-term relationships with patients instead of rushing through high-volume visits. You will work closely with an interdisciplinary team to provide comprehensive, coordinated care for seniors with complex medical, social, and functional needs. This role is especially well suited for a physician who has experience working within an interdisciplinary team structure and is comfortable partnering with, supporting, or leading Nurse Practitioners and Physician Assistants. This is not a traditional primary care model. You will see fewer patients each day, spend more meaningful time with them, and help guide care across the full picture of their lives, including medical needs, home environment, family support, functional status, and long-term goals. There are no hospital rounds. The focus is on proactive, relationship-based care with regular weekday hours, a collaborative team structure, and the opportunity to make a visible difference in the lives of seniors and their families.

Requirements

  • Physician trained in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics.
  • Successful completion of an ACGME-accredited residency program.
  • Active state physician license without restrictions.
  • State CDS certification and DEA registration.
  • Eligible to provide services to Medicare and Medi-Cal participants.

Nice To Haves

  • Board certification preferred.
  • CPR and First Aid certification preferred upon hire, or ability to obtain within required timeframe.
  • Experience in geriatric care, senior care, value-based care, or complex primary care strongly preferred.
  • Experience working in an interdisciplinary team model preferred.
  • Experience leading or collaborating closely with Nurse Practitioners or Physician Assistants preferred.

Responsibilities

  • Perform comprehensive in-person assessments for senior participants.
  • Lead and partner with the interdisciplinary care team to develop individualized care plans.
  • Provide thoughtful primary care with an average of approximately eight patients per day.
  • Coordinate care transitions, including acute, short-term, and long-term placements.
  • Partner with Nurse Practitioners, Physician Assistants, nursing, social work, therapy, and other care team members.
  • Participate in regular care planning discussions focused on whole-person health.
  • Support home-based care needs through scheduled home visits as part of the role.
  • Participate in a rotating phone-only call schedule a few nights or weekends per month.
