About The Position

We are a unique, rapidly expanding primary care provider focused on transforming healthcare for seniors. We are seeking innovative, entrepreneurial-minded individuals with a strong work ethic and a passion for improving lives. The Primary Care Physician (PCP) at ChenMed is responsible for delivering high-quality care to a panel of up to 450 patients, focusing on accountability for outcomes and health coaching. The PCP will work collaboratively with care teams to achieve performance goals and ensure compliance with healthcare guidelines. This role requires a commitment to patient-centered care and the ability to lead and inspire others in a positive culture. The candidate will be responsible for providing care for patients with acute and chronic illnesses encountered in the older adult patient.

Requirements

An MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is required.
A current, active State Medical License for the state(s) in which the candidate will be working is mandatory.
Proficiency in Microsoft Office Suite products including Word, Excel, PowerPoint, and Outlook.
Experience with evidence-based medicine and a commitment to continuous improvement in patient care is essential.

Responsibilities

Collaborate cross-functionally to simplify and prioritize care for patients, ensuring actionable behavior changes.
Lead efforts to manage their care team effectively, ensuring high-quality patient care and compliance with guidelines.
