About The Position

HealthFirst Family Care Center is seeking a compassionate, collaborative Primary Care Physician to join their team and provide high-quality, patient-centered care to individuals and families in the communities they serve. As a Federally Qualified Health Center, HealthFirst provides comprehensive primary care to patients throughout the Lakes Region of New Hampshire. Their providers care for a diverse patient population and work alongside an experienced, multidisciplinary team committed to improving health, removing barriers to care, and building lasting relationships with their patients.

Requirements

  • MD or DO from an accredited medical school
  • Completion of an accredited residency in Family Medicine or Internal Medicine
  • Board certification
  • Current or eligible New Hampshire medical license
  • Current DEA registration
  • Strong communication and collaboration skills
  • A commitment to compassionate, respectful, patient-centered care
  • Graduate of an accredited Medical School and completion of accredited medical residency in either Family Practice or Internal Medicine.
  • New Hampshire license, Board certified with current DEA.
  • Demonstrates effective communication skills, ability to interact within a team setting, and demonstrate professional behavior and appearance.
  • Displays a courteous and supportive manner with staff as well as a competent, confidential, and compassionate manner with patients.
  • Ability to assist clinical staff in interpretation of medical charts, monitors, and other pertinent patient medical data.
  • Computer skills and documentation in electronic medical records is necessary.
  • Collaborates with members of other disciplines, community agencies, school personnel, and hospital staff in meetings the needs of the community.
  • Acts as a consultant, resource and support for the clinical staff.

Responsibilities

  • Provide comprehensive primary care focused on prevention, wellness, and management of acute and chronic conditions
  • Develop individualized treatment plans based on each patient's medical and psychosocial needs
  • Educate and counsel patients to help them actively participate in their health and wellness
  • Collaborate with physicians, advanced practice providers, nursing staff, behavioral health professionals, and other members of the care team
  • Participate in quality improvement and population health initiatives
  • Serve as a clinical resource and support to members of the care team
  • Maintain timely, accurate documentation in the electronic health record
  • Participate in the clinic's on-call rotation as applicable
