About The Position

Upperline Health is seeking a Primary Care Physician (MD/DO) for its Port Orange, FL Clinic. This role focuses on practicing thoughtful, value-based care medicine for medically complex patients. It is a clinic-based position within an integrated specialty and primary care organization, emphasizing chronic disease management, prevention, and long-term patient outcomes. The position offers predictable hours (Monday-Friday, 8am-5pm) with no nights or weekends, a manageable patient volume, and light outpatient phone call only. It provides an outpatient-only opportunity to practice within a multidisciplinary care model, coordinating patient care with specialists like Podiatric Physicians and Surgeons, Endocrinologists, Advanced Practice Providers, Care Navigators, Pharmacists, Dieticians, and Social Workers. This role is designed for physicians who want to practice medicine differently, focusing on impacting long-term outcomes rather than just treating symptoms, and offers career growth, leadership development, and partnership opportunities.

Requirements

  • Medical Doctor or Doctor of Osteopathic Medicine required
  • Successful completion of accredited Internal Medicine or Family Medicine residency program required
  • Board certification or board eligibility required
  • Medical license required – active license preferred or the ability to obtain within applicable timeframe
  • Possession of DEA registration or eligibility required
  • BLS certification and maintenance

Responsibilities

  • Provide comprehensive primary care to a defined patient population, many with chronic conditions such as diabetes, vascular disease and mobility limitations.
  • Diagnose conditions, develop treatment plans, and prescribe medications and therapies as appropriate, working with Upperline Health specialists and care teams to proactively manage conditions, reduce complications, and improve quality of life.
  • Conduct patient assessments through medical history, physical exams, and diagnostic evaluation.
  • Order, review, and interpret laboratory tests and diagnostic imaging.
  • Coordinate and manage referrals to specialists and follow up on patient outcomes.
  • Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
  • Maintain timely, accurate clinical documentation in the electronic health record.
  • Comply with clinical policies, quality standards, and regulatory requirements.
