About The Position

The organization is seeking a Board-Certified or Board-Eligible Primary Care Physician to join its growing outpatient primary care team in Naugatuck, Connecticut. This role includes comprehensive outpatient primary care across the lifespan, management of acute and chronic medical conditions, preventive care, wellness visits, and health screenings. Chronic disease management including hypertension, diabetes, asthma, COPD, and other common conditions, coordination of care with specialists and hospital-based providers, and collaboration with advanced practice providers and multidisciplinary support staff are key aspects of this position. There are also opportunities to participate in academic, teaching, and quality improvement initiatives. Requirements

  • M.D. or D.O. degree
  • Completion of an accredited Family Medicine residency program
  • Board Certified or Board Eligible in Family Medicine
  • Eligible for Connecticut State Medical Licensure
  • Active or eligible DEA registration
  • BLS certification required
  • Strong clinical judgment and diagnostic abilities
  • Excellent communication and interpersonal skills
  • Commitment to patient-centered, team-based care
  • Ability to establish long-term patient relationships
  • Interest in collaboration, education, and quality improvement

Responsibilities

  • Provide comprehensive outpatient primary care services to pediatric, adult, and geriatric patients
  • Diagnose and manage acute illnesses and chronic medical conditions
  • Conduct annual wellness visits, preventive screenings, and routine physical examinations
  • Develop individualized treatment plans and provide longitudinal patient care
  • Order, review, and interpret laboratory and diagnostic studies
  • Coordinate patient care with specialists and hospital-based teams
  • Maintain timely and accurate documentation within the electronic medical record (EMR)
  • Participate in shared phone-call coverage with minimal burden
  • Collaborate effectively with physicians, APPs, nurses, and care coordinators
  • Participate in quality improvement initiatives and organizational programs
