About The Position

HealthDrive delivers primary/post-acute care to residents in long-term care, skilled nursing and assisted living facilities. We bring vital healthcare to patients in need by way of our practitioners who are committed to providing care with dignity and excellence. We are seeking a highly skilled and collaborative Primary Care Physician to join our growing telehealth team supporting patients in skilled nursing facilities (SNFs). This is a unique opportunity to deliver high-impact care remotely while partnering closely with on-site clinical teams and leveraging innovative diagnostic technology. In this role, you will conduct on-demand telemedicine visits, interpret diagnostic testing results, and guide care plans in collaboration with facility staff. You will also play a key role in clinical oversight by performing structured evaluations of nurse practitioners to ensure high-quality, patient-centered care.

Requirements

  • MD or DO degree from an accredited medical school
  • Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
  • Licensed or willing to obtain licensure in New Jersey and Rhode Island; Connecticut licensure may be required in the future
  • Demonstrated ability to collaborate effectively with interdisciplinary teams
  • Strong clinical judgment and comfort interpreting diagnostic data remotely

Nice To Haves

  • Experience working with geriatric populations and/or in skilled nursing facilities strongly preferred
  • Prior telehealth experience is a plus

Responsibilities

  • Conduct remote primary care and consultative visits with patients in skilled nursing facilities via telemedicine
  • Review and interpret diagnostic testing results obtained through mobile clinical technology (e.g., diagnostic carts)
  • Develop and communicate treatment recommendations in collaboration with on-site medical staff
  • Partner with interdisciplinary care teams to ensure continuity and quality of care
  • Document all patient encounters and recommendations in a timely and compliant manner
  • Perform 5–10 structured clinical assessments per day (on designated days) for nurse practitioners
  • Conduct comprehensive chart reviews and clinical audits
  • Observe patient interactions and care delivery (virtually)
  • Provide constructive feedback, coaching, and guidance to support clinical excellence
  • Ensure adherence to evidence-based practices, organizational standards, and regulatory requirements
