About The Position
We are seeking a California licensed Primary Care Physician for a PACE (Program of All-Inclusive Care for the Elderly) program. This program allows seniors to continue living in their homes and communities. The organization provides comprehensive medical services with a focus on treating the whole person. A collaborative interdisciplinary team (IDT) approach is used to evaluate and guide participant care, reflecting the organization's values, team culture, and mission.
Requirements
- Active California physician license with no restrictions.
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Board Certification in Internal Medicine or Family Practice.
Responsibilities
- Perform in-person comprehensive assessments for participants.
- Coordinate with the IDT to develop a care plan for each participant.
- Oversee all care of participants in nursing homes and hospitals, including admits and discharges for acute, short-term, and long-term placement, at the direction of the Medical Director.
- Utilize medical specialists to ensure effective care coordination.
- Provide direction and support to Nurse Practitioners and other members of the IDT.
