About The Position

Upperline Health is redefining primary care for medically complex patients through a value-based, outcomes-driven care model. We are seeking a Primary Care Physician (MD/DO) for our Daytona Beach, FL Clinic who wants to practice thoughtful, value-based care medicine. This is a clinic-based role within an integrated specialty and primary care organization, focused on chronic disease management, prevention, and long-term patient outcomes. The role offers predictable hours and a sustainable workload (Monday – Friday, 8am – 5pm, no nights and weekends) with flexible scheduling options. It involves manageable patient volume and a sustainable pace, with no hospital call and only light outpatient phone call. This position provides a unique outpatient-only opportunity to practice within a multidisciplinary care model designed to proactively manage primary care needs along with Chronic Disease Management. Upperline Health providers coordinate patients’ care among a team of specialists for integrated treatment that addresses patients’ immediate and long-term health needs. We put patients at the center of value-based care. This is a unique role designed for physicians who want more than traditional primary care.

Requirements

  • Medical Doctor or Doctor of Osteopathic Medicine required
  • Successful completion of accredited Internal Medicine or Family Medicine residency program required
  • Board certification or board eligibility required
  • Medical license required – active license preferred or the ability to obtain within applicable timeframe
  • Possession of DEA registration or eligibility required
  • BLS certification and maintenance

Responsibilities

  • Provide comprehensive primary care to a defined patient population, many with chronic conditions such as diabetes, vascular disease and mobility limitations.
  • Diagnose conditions, develop treatment plans, and prescribe medications and therapies as appropriate, working with Upperline Health specialists and care teams to proactively manage conditions, reduce complications, and improve quality of life.
  • Conduct patient assessments through medical history, physical exams, and diagnostic evaluation.
  • Order, review, and interpret laboratory tests and diagnostic imaging.
  • Coordinate and manage referrals to specialists and follow up on patient outcomes.
  • Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
  • Maintain timely, accurate clinical documentation in the electronic health record.
  • Comply with clinical policies, quality standards, and regulatory requirements.
