You will serve as the clinical leader of an interdisciplinary team, collaborating with nurses, social workers, and therapists to create comprehensive care plans that keep patients healthy, independent, and in their homes. Manageable Caseload: See only 8–10 patients per day, allowing for deep clinical assessment and relationship building. True Work-Life Balance: Maintain a standard 8:00 AM – 5:00 PM, Monday through Friday schedule. Enjoy your weekends and evenings with a phone-only, limited on-call rotation. No Hospital Rounding: Focus your expertise entirely on community-based care. Requirements

• Completion of an accredited residency in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics.
• Active, unrestricted state physician license, CDS certification, and DEA registration.
• Ability to maintain CPR and First Aid certification is required.
• Must be eligible to serve Medicare and Medi-Cal participants.

Nice To Haves

• Board Certification is preferred.
• 2–5 years of primary care experience is ideal, but passionate new graduates are encouraged to apply.

Responsibilities

• Conduct thorough in-person assessments and provide high-touch primary care.
• Lead Interdisciplinary Team (IDT) meetings to craft and execute personalized, holistic care plans.
• Oversee transitions of care, including acute episodes and long-term placement management.
• Act as the primary advocate for your patients within the Medicare and Medi-Cal systems.
