This is a full-time outpatient opportunity within a physician-led, value-based care model focused on senior populations, prevention, long-term relationships, and improved health outcomes. POSITION OVERVIEW The Primary Care Physician manages a defined patient panel while leading a multidisciplinary care team in a collaborative outpatient setting. This role emphasizes accountability for patient outcomes, chronic disease management, preventive care, and cost-effective treatment planning. SCHEDULE • Monday through Friday • Full-time outpatient clinic setting • No traditional call responsibilities COMPENSATION • Salary Range: $214,700 – $306,714 • Performance-based incentive opportunities available • Compensation based on experience and market KEY RESPONSIBILITIES • Provide comprehensive primary care to adult and geriatric patients • Manage a patient panel of approximately 450 patients • Diagnose, treat, and manage acute and chronic conditions • Lead and collaborate with care team members including care promoters, facilitators, and coordinators • Engage patients in preventive care and health coaching • Coordinate care across settings including hospital, SNF, LTC, and home-based transitions • Maintain accurate documentation and participate in risk and quality programs • Support patient engagement and improved outcomes through education and follow-up WHY THIS OPPORTUNITY • Value-based care model focused on outcomes over volume • Smaller patient panels allowing deeper patient relationships • Physician-led organization with strong clinical support • Emphasis on prevention, chronic care, and population health • Career development and advancement opportunities

Requirements

  • MD or DO required
  • Board Certified or Board Eligible in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required
  • Active Missouri medical license or ability to obtain required
  • Active DEA license required
  • BLS certification required within 90 days of employment
  • Strong interest in value-based care and population health required
  • Ability to manage patient panels and drive clinical outcomes
  • Strong leadership, collaboration, and communication skills required

  • Experience in primary care, geriatrics, or internal medicine preferred
