About The Position

The Physician provides comprehensive outpatient care focused on preventive medicine, chronic disease management, continuity of care, and improving overall patient health outcomes. This role emphasizes building meaningful patient relationships while delivering high-quality, value-based care in a collaborative, team-oriented environment. You will report directly to the Area Medical Director. This practice will be onsite at our Conviva Pompano East clinic.

Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active, unrestricted medical license in the state of practice (or willingness to obtain)
• Current, unrestricted DEA license

Nice To Haves

• Two or more years of clinical practice experience
• Experience managing Medicare Advantage patient populations
• Knowledge of Medicare regulations and coverage guidelines
• Familiarity with HEDIS quality measures and value-based care initiatives
• Bilingual Haitian Creole and English preferred

Responsibilities

• Provide comprehensive outpatient care focused on preventive medicine, chronic disease management, continuity of care, and improving overall patient health outcomes.
• Build meaningful patient relationships.
• Deliver high-quality, value-based care in a collaborative, team-oriented environment.
• See approximately 12–15 patients per day, allowing for a more personalized approach to care.
• Spend time focused on direct patient care, supported by a dedicated clinical and operational team.
• Work within a value-based care model designed to improve patient outcomes and enhance the provider experience.
• Participate in Humana/Senior Bridge's Tuberculosis (TB) screening program.
