We are seeking a dedicated Primary Care Physician (PCP) to join our growing team in Baltimore following a physician transition. Our care model is designed around smaller patient panels, interdisciplinary collaboration, and proactive wellness-focused care for Medicare-eligible adults and underserved senior communities. Physicians are empowered to build lasting relationships with patients while supported by an experienced clinical and operational team committed to improving outcomes, reducing barriers to care, and strengthening the communities we serve.

Requirements

MD or DO from an accredited medical school
Board-certified or board-eligible in Internal Medicine or Family Medicine
Active or eligible Maryland medical license

Nice To Haves

Technologically savvy physician strongly preferred
Experience in value-based care is a significant plus

Responsibilities

Practice meaningful medicine with the time, support, and resources needed to truly impact patients’ lives.
Provide proactive wellness-focused care for Medicare-eligible adults and underserved senior communities.
Build lasting relationships with patients.
Collaborate with an interdisciplinary care team including nurses, MAs, case managers, social workers, and community outreach specialists.
Participate in continuous professional development and training in value-based care.
