About The Position

Daymark is partnering with leading healthcare providers across New England to bring supportive oncology care into the home for patients with cancer. As Daymark's Primary Care Physician, Supportive Oncology, you will be one of the clinical leads for that work and one our care teams turn to for guidance. You will combine direct patient care with clinical oversight and be a visible leader in our provider partnerships. You will support each partnership clinically, enhancing how we work alongside a partner's primary care physicians and oncologists and making sure the care we deliver in the home is care that partners trust with their patients. You will see patients virtually, partner with our Nurse Practitioners, help set the standard for how we document and communicate the care we deliver, and serve as a steward of Daymark's clinical program. You should be comfortable operating as a player/coach – rolling up your sleeves on direct care while shaping the processes that let partnerships thrive.

Requirements

  • Board-certified physician (MD or DO) who completed residency training in internal medicine, family medicine, or geriatric medicine.
  • Able and willing to enroll in Medicare under Daymark and complete enrollment promptly after start date.
  • Active, unrestricted medical licenses in both Massachusetts and Rhode Island.
  • Active DEA registration and eligible to register in both states.
  • Eligible to enroll as a Medicare provider and not excluded, debarred, or sanctioned under any federal or state health care program.
  • At least 5 years of practice experience including substantial direct patient care.
  • Meaningful experience caring for patients with cancer, or in palliative care, hospice, or serious illness care.
  • Supervised or collaborated with Nurse Practitioners and comfortable sharing accountability for team clinical performance.
  • Practiced in a value-based or shared savings environment (ACO, Medicare risk arrangement, or similar) and understand how quality measurement, care gap closure, and documentation affect program performance.
  • Comfortable caring for patients and at ease meeting patients where they live.
  • Willing to travel as needed within Massachusetts and Rhode Island.
  • Tech savvy and comfortable caring for patients both in person and virtually over video and web-based platforms.
  • Comfortable with ambiguity and with building something that may not exist yet.
  • Communicate well with oncologists, primary care physicians, and patients and families.
  • Basic Life Support (BLS) certification.
  • Compliance with Daymark's Employee Vaccination and Testing policy.
  • Proof of a valid driver's license (for roles requiring in-home visits).

Nice To Haves

  • Board certification in Hospice and Palliative Medicine is strongly preferred.
  • Direct experience with home-based care is preferred.

Responsibilities

  • Provide direct patient care, including comprehensive initial visits, managing complex cases, supporting post-discharge transitions, and responding to clinical escalations.
  • Conduct assessments, place orders, prescribe medications, and document care in the EHR.
  • Partner with regional leadership to refine SOPs, care pathways, and risk stratification approach.
  • Contribute clinical expertise to the design of new programs and workflows.
  • Serve as a clinical representative to oncology practices, primary care physicians, health systems, ACOs, and health plan partners.
  • Bring clinical credibility to conversations with partner practices.
  • Ensure Daymark meets clinical and quality obligations in each partnership, including quality gap closure.
