About The Position

AllCare.ai delivers coordinated, in-facility care — including Primary Care, Psychiatry, Podiatry, Diagnostics, Wound Care, and Chronic Care Management — supported by a 24/7 medical concierge. Our platform streamlines documentation and compliance so providers can focus on delivering high-quality care to seniors. We are seeking a Primary Care Physician (MD/DO) to provide in-facility care to residents in assisted living communities across the Inland Empire, CA. This role involves mobile in-facility visits with clustered routes to minimize travel time. This is a part-time position starting at a minimum of 1 day per week (Monday through Friday), with potential to expand based on patient panel growth. This position is available as W-2 or 1099 — we are happy to accommodate what works best for you.

Requirements

• Active California medical license (MD or DO) in good standing
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
• Active DEA registration
• Valid driver's license and reliable vehicle

Nice To Haves

• Medicare credentialing preferred
• Experience in geriatric or senior care settings preferred

Responsibilities

• Provide comprehensive primary care to seniors in assisted living and RCFE communities in the Inland Empire area
• Conduct in-facility patient visits and telehealth consultations as needed
• Manage chronic conditions, preventive care, and acute concerns
• Collaborate with our care team including NPs, specialists, and facility staff
• Maintain accurate and timely documentation in compliance with regulatory requirements
