About The Position

Humana's Primary Care Organization is a provider of senior-focused, value-based care, with more than 340 centers across 15 states under the CenterWell and Conviva brands. This is a full-time, on-site, patient-facing role. You report to the Market Chief Medical Officer (CMO), or Associate Medical Director (AMD), and are responsible for both direct patient care and clinical leadership for the Anderson, SC clinic. About the Role You will provide primary care for senior patients in a value-based care model while leading a team of clinicians to lead quality, outcomes, and patient experience. You will balance patient care with leadership responsibilities, supporting clinicians, improving performance, and advancing a patient-centered model of care.

Requirements

  • MD or DO from an accredited program
  • Board Certified in Family Medicine, Internal Medicine, or Geriatrics
  • Active, unrestricted medical license (or ability to obtain) in the state of practice
  • 8+ years of clinical experience
  • 2+ years of leadership or project leadership experience

Nice To Haves

  • Experience managing Medicare Advantage patient panels
  • Experience in value-based care or coordinated care environments
  • Experience supervising Advanced Practice Providers (NPs/PAs)
  • Active, unrestricted DEA license
  • Medicare and Medicaid provider enrollment

Responsibilities

  • Provide primary care for senior patients, including those with complex and chronic conditions
  • See patients and manage ongoing care, typically spending 50–80% of your time in direct patient care
  • Complete accurate documentation to support care and outcomes
  • Manage a clinical inbox, including labs, messages, and follow-ups
  • Coordinate care with specialists and interdisciplinary care teams
  • Apply Medicare risk adjustment principles through accurate condition documentation
  • Lead a team of Physicians, Nurse Practitioners, and Physician Assistants
  • Coach clinicians using data, performance dashboards, and patient outcomes
  • Participate in hiring, onboarding, and retention of clinical staff
  • Provide performance feedback, complete evaluations, and support professional growth
  • Foster a culture of collaboration, accountability, and continuous improvement
  • Improve performance in patient experience, quality, and clinical outcomes
  • Monitor and improve important metrics such as: Net Promoter Score (NPS), HEDIS and Medicare Stars measures, Total cost of care and avoidable utilization
  • Conduct chart reviews to identify care opportunities and guide improvement
  • Partner with operational leaders to align on clinical and business goals
  • Collaborate on provider schedules, patient access, and care delivery strategies
  • Support performance management, team engagement, and clinic operations
  • Contribute to consistent communication and aligned decision-making across teams
