Requirements

  • Unrestricted medical licensure in the state of Indiana or able to obtain prior to start
  • Board certified or board eligible as a Family Nurse Practitioner (ANCC/AANP) or a Physician Assistant (NCCPA)
  • Active and unrestricted DEA license or ability to obtain prior to start

Responsibilities

  • Provide comprehensive primary care to a diverse patient population
  • Collaborate with interdisciplinary care teams
  • Perform in-office procedures (optional)
  • Participate in quality and population health initiatives
  • Educate and engage patients in their care
