About The Position

Acts is seeking a Primary Care Office Nurse (LPN) to support residents’ health and well-being by delivering high-quality, person-directed care and nursing services within the Primary Care Office. The role contributes to Acts’ Signature Well-Being Model through interdisciplinary care, proactive health monitoring, and facilitating residents’ access to well-being resources. The nurse collaborates closely with the Nurse Practitioner and broader wellness team to promote a holistic, resident-centered approach that supports residents’ independence, dignity, and ability to thrive in place.

Requirements

• Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing.
• At least two years of clinical nursing experience.
• Current CPR, AED, and first aid certification, or ability to obtain it within three months of employment.

Nice To Haves

• Clinical nursing experience in senior living, primary care, or a wellness-focused environment.
• Prior experience in office management, care coordination, or leadership roles.
