The Deborah Smith Walsh Recuperative Care Center (“RCC”) provides short-term, residential care to people who are homeless and sick, but not sick enough to be in a hospital. This program provides guests with much-needed care at a critical time in their rehabilitation, and an alternative to trying to recuperate on the streets. The Center staff will identify clients who are unconnected to care and have a history of multiple emergency department visits. By coordinating medical care, short-term residential care, and other services, the RCC provides a vital service to support medically and psychiatrically complex homeless adults. Hours: Part-time role - Weekends Only SUMMARY: Primary Care Nurse (RN/LPN) provides patient care within the ambulatory setting, utilizing assessment and teaching skills to foster patient health. The PC Nurse tracks patients at risk, collaborates with medical providers, performs POC testing, administers medications/immunizations, and responds to the health education needs of patients and families. The PC Nurse provides information and direction to support the Team CA staff to assure clinic flow and that patient needs are met. Participates in LCHC community outreach efforts, as appropriate and as assigned. This may include serving as a health consultant for the Lynn Public Schools, local day care centers, after school program, day camps or other community related organizations, providing such services as programmatic consultation, preparation/review of health-related policies and documentation or participating in health screenings, health education programs or counseling at health fairs and other similar community events.
