About The Position

Hartford HealthCare at Home is seeking a Primary Care Nurse (RN) to provide personalized care to patients in their homes. This role offers autonomy and the opportunity to utilize advanced technology and evidence-based practices. As part of Hartford HealthCare, the largest provider of homecare services in Connecticut, you will contribute to a mission that has been active for over 115 years, focusing on a Person-Centered Care Model and integrated patient support. This position covers Union, Ashford, Eastford, Chaplin, Hampton, and surrounding towns.

Requirements

• Registered Nurse with Valid CT RN license
• One year of RN experience
• Valid driver’s license and reliable transportation required
• Computer proficiency required

Nice To Haves

• Homecare experience preferred

Responsibilities

• Perform comprehensive assessments to establish an individualized care plan.
• Facilitate communication between the patient and the interdisciplinary team.
• Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.
• Utilize state-of-the-art technology to document skilled services provided.
