Primary Care Nurse Practitioner - Stockton / Modesto / Amador
About The Position
AllCare.ai delivers coordinated, in-facility care (Primary Care, Psychiatry, Podiatry, Diagnostics, Wound Care, CCM) with a 24/7 concierge. Our platform automates charting and compliance so you can focus on high-quality senior care. We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador. What You’ll Do Routine & urgent assessments; chronic disease management Post-discharge follow-ups, med reconciliation, labs/diagnostics Coordinate with facility staff, specialists, families/POAs Same-day documentation; CMS/regulatory compliance Schedule & Setting Full-time position with a structured weekday schedule. Providers deliver on-site care across Stockton, Modesto, and Amador with strategically clustered routes, so you spend more time with patients and less time on the road.
Requirements
- Active CA NP & RN licenses
- Active furnishing license
- NPI, DEA, BLS
- EMR proficiency
Nice To Haves
- Geriatrics or facility-based experience preferred
