This role involves performing history and physical examinations, documenting visits, educating patients, managing conditions, ordering tests, developing treatment plans, prescribing therapies, making referrals, and collaborating with a supportive medical and management team.

Requirements

  • Must have completed an accredited NP/PA program and be registered by the NYS Department of Education
  • 3 years or more experience practicing as a Nurse Practitioner or Physician assistant
  • Proven ability to work in a team environment

Responsibilities

  • Performing history and physical examinations
  • Documenting visits in electronic medical record in a timely manner
  • Educating patients on their health conditions in a way they can understand
  • Following up with patients more frequently for effective condition management
  • Utilizing your support team to complete standing orders
  • Ordering diagnostic tests based on assessments, developing treatment plans, prescribing drugs and other therapies as needed
  • Making referrals as necessary, consulting with supervising medical provider when appropriate
  • Maintaining patient confidentiality
  • Participating in meetings
  • Working collaboratively with other members of the medical team
  • Collaborating with the management team on quality improvement initiatives
  • Having an open mind to feedback, different perspectives, reflection, and growth
