Part-Time Primary Care Nurse Practitioner
About The Position
AllCare.ai is seeking a part-time Primary Care Nurse Practitioner to provide on-site care to Assisted Living/RCFE residents in Eureka and the surrounding Humboldt County area. The role includes in-person care three days per week on a structured weekday schedule, with routes clustered to minimize travel and maximize patient-care time. Occasional telehealth visits provide additional flexibility. The practitioner delivers routine and urgent assessments, chronic disease management, post-discharge follow-ups, medication reconciliation, and labs and diagnostics. The role also coordinates with facility staff, specialists, and residents’ families or powers of attorney, and requires same-day documentation and CMS/regulatory compliance. AllCare.ai provides coordinated in-facility services, including primary care, psychiatry, podiatry, diagnostics, wound care, and chronic care management, supported by a 24/7 concierge. Its platform and modern tools are intended to automate charting and compliance and reduce administrative work. Compensation is an annual range of $115,000–$130,000, based on experience, schedule, and patient volume, with additional earning opportunities based on coverage and panel needs. The position is W-2, and mileage is reimbursed per policy; final terms are outlined in the employment agreement.
Requirements
- Active California Nurse Practitioner license.
- Active California Registered Nurse license.
- Active furnishing license.
- NPI.
- DEA registration.
- BLS certification.
- EMR proficiency.

Responsibilities
