About The Position
Prime Healthcare Illinois Medical Center is seeking a dedicated and compassionate Nurse Practitioner to join their growing Primary Care team at St. Mary of Nazareth in Chicago, IL. This is a rewarding full-time opportunity to provide comprehensive, patient-centered care in a well-established outpatient setting. The position involves working 40 hours per week in an outpatient primary care clinic, seeing patients for preventive care, chronic disease management, and acute visits. The Nurse Practitioner will collaborate closely with a team of physicians, APPs, and clinical support staff and manage their own patient panel with opportunities for continuity of care, with minimal call responsibility.
Requirements
- Licensed and certified as a Nurse Practitioner (NP) in the state of Illinois
- Board-certified in Family Medicine, Adult-Gerontology, or related field
- Strong clinical assessment, communication, and documentation skills
- Commitment to providing high-quality, compassionate care
Nice To Haves
- Ukrainian & Spanish Proficiency Preferred
- Prior experience in primary care preferred but not required
