About The Position

Infinite Medical P.C. is hiring a full-time Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care to residents in skilled nursing and long-term care facilities in Staten Island, New York. The position is intended for a provider who values meaningful patient relationships and collaborative care. Infinite Medical P.C. is a nationwide network of advanced practice providers and specialty clinicians focused on proactive, personalized care for residents in skilled nursing and long-term care settings, in partnership with MedElite Healthcare Management Group.

Requirements

Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
New York state-specific license in good standing.
At least 6 months of experience in long-term care or a skilled nursing facility.
Strong communication, clinical, and critical-thinking skills.
Passion for elder care and person-centered healthcare.

Responsibilities

Provide primary care services to residents in skilled nursing and long-term care facilities.
Perform physical examinations and medical history assessments.
Diagnose and treat chronic and acute health conditions.
Order and interpret diagnostic tests and laboratory results.
Develop and adjust individualized care plans.
Prescribe medications and monitor treatment effectiveness.
Collaborate with facility staff and interdisciplinary teams.
Educate patients and families about treatment options and care goals.
Help ensure a safe, respectful, and supportive environment for residents.
