Adult-Gerontology Primary Care Nurse Practitioner Fellowship Program Wyckoff Heights Medical Center (WHMC) is a 350-bed teaching hospital located in an ethnically diverse area of Brooklyn, New York. As a safety-net hospital, WHMC wishes to expand its mission of delivering high-quality care through education in a safe environment by increasing accessibility to primary care. By fostering a healthier community, we wish to expand our footprint in ambulatory care while simultaneously improving clinical outcomes for our patients through increasing satellite points of care delivery, thereby reducing clinic wait-times, hospital readmission rates, and overall patient and provider satisfaction. To accomplish our institution's strategic goal, we are launching our inaugural Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) Fellowship Program. Beginning in September 2025, this 12-month immersive educational curriculum seeks to provide bespoke primary care training and experience to newly-graduated Nurse Practitioners (NPs) seeking to further their expertise and broaden their knowledge to integrate evidence-based application into clinical practice. Through a series of clinical rotations, didactic series, and mentorship with practicing Primary Care NPs, this program follows in the footstep of our accredited Pediatric Nurse Practitioner (PNP) Fellowship Program, which has been in operation for over eight years. Program Includes: Precepted primary care educational curriculum Mentored independent clinics Specialty rotations Didactic sessions
