About The Position

This is a full-time 5-day/week Nurse Practitioner (NP) or Physician Assistant (PA) position focused on primary care within skilled nursing and post-acute facilities. The role offers a flexible round-and-go model with significant clinical autonomy. The position includes dedicated night call coverage, structured onboarding, clinical mentorship, and multidisciplinary support. There are also opportunities for leadership and professional growth.

Requirements

• New York NP or PA license and DEA (or eligible)
• Post-acute care experience required.
• Minimum of 1 year primary care experience.
• Strong clinical assessment and independent care-management skills.

Nice To Haves

• Background in geriatrics, hospital medicine, or emergency medicine.
