About The Position

A respected healthcare organization in Central New York is seeking a Licensed Practical Nurse (LPN) to join its Primary Care team. Opportunities are available in both Oswego and Fulton, NY. This full-time, day-shift position offers the opportunity to provide patient-centered care in an outpatient setting while working alongside physicians, advanced practice providers, nurses, and other members of the healthcare team.

Requirements

Current New York State LPN license.
Current CPR certification.
Strong communication, organization, and patient-service skills.
Ability to work effectively within a collaborative healthcare team.

Nice To Haves

One year of experience in a medical office or outpatient setting preferred.
Proficiency in medication administration and phlebotomy preferred.

Responsibilities

Assist in providing and evaluating patient care as a member of the primary care team.
Support physicians and other providers with the delivery of high-quality patient care.
Administer medications in accordance with established policies, procedures, and standards of care.
Prepare and administer medications through appropriate routes and maintain accurate documentation.
Observe patients for reactions to medications, treatments, or changes in condition and report concerns appropriately.
Assist with patient mobility and maintain a safe, comfortable clinical environment.
Follow infection prevention, aseptic technique, and equipment sterilization guidelines.
Maintain appropriate levels of clinical supplies and routinely check emergency equipment.
Accurately document patient care and utilize electronic systems as required.
Communicate effectively and compassionately with patients, families, providers, and other members of the healthcare team.
