With a growing network of sites and a truly integrated model of care, we’re seeking a Primary Care Physician (MD/DO) who is passionate about equity, collaboration, and whole-person care.

Requirements

• MD or DO from an accredited medical school
• Active New York State license and DEA
• Board Certified or Board Eligible (within 18 months)

Responsibilities

• Provide full-scope primary care including preventive, acute, and chronic disease management
• Lead and collaborate within a multidisciplinary care team
• Perform exams, minor procedures, and prescribe per NYS guidelines
• Coordinate care across services, including referrals, labs, and follow-up
• Engage in quality improvement and population health initiatives
• Participate in community outreach and program development
